Telemed is a provider of Telehealth, Minute Clinic and IV Hydration Therapy. 

Our Telehealth Services is a way to visit with healthcare providers, such as your doctor or nurse practitioner remotely by means of telecommunications technology. 

You can talk to your provider from any place, including your home. You don’t go to a clinic or hospital. 

Prior to accessing a Telehealth Consultation, you represent and warrant that you are at least eighteen years of age and possess the legal right and ability, on behalf of yourself or a minor child of whom you are a parent or legal guardian, to agree to these Terms and Conditions and abide by the obligations hereunder. You are responsible for providing and maintaining, at your own risk, option, and expense, appropriate software and hardware capabilities (consistent with any technical, quality, or other requirements described on the Site) to enable use of the Site, including, but not limited to, a computer or mobile device with internet access. You are solely responsible for any fees, including internet connection or mobile fees that you incur while using the Site or a Telehealth Consultation.

This consultation and treatment are subject to the terms and conditions of this agreement.

Not all consultations will lead to the offer of a prescription.

You undertake to complete the consultation questionnaire relating to your condition contained on this website truthfully and honestly, disclosing all relevant information truthfully and to the best of Your knowledge.

We warrant and undertake that your consultation questionnaire will only be viewed by the medical professional and his staff for the purpose of treating your medical condition and by the Pharmacist who dispenses any medication pursuant to any prescription that the doctor may issue.

We are committed to protecting and respecting your privacy and to using technology to enhance your online security. Before you use certain areas of this website, We ask You to register on it by requiring You to enter Your personal information, including "sensitive personal data. We will not release to any third party any personally identifying information obtained in this way, without Your prior written consent. We will only use the information that We collect about You lawfully. In respect of the aforementioned "sensitive personal data" by accepting this agreement You grant Us explicit consent to collect such data.

We will limit the collection and use of such information to the minimum required to deliver, develop and improve Our services and other opportunities and the efficient administration of the medical professional’s consultation

Dispute Resolution.

Telemed will try work in good faith to resolve any issue you have with Site, including services ordered or purchased through the Site, if you bring that issue to the attention of our customer service department. However, we realize that there may be rare cases where we may not be able to resolve an issue to a customer’s satisfaction. In the event we cannot resolve a dispute between us, you agree that all matters related to any use or access of the Site, the Services or any other Telemed site will be governed by the laws of the State of Florida, without regard to its conflicts of laws rules. You hereby waive any objections to such jurisdiction or venue. Any claims or controversies arising out of this Agreement, and/or the related use and access of the Site, the Services or any other Telemed site, shall be exclusively in the state courts of the Seventeenth Judicial Circuit in Broward County, Florida.

Payments and Recurring Billing.

Your Payment Information. 

 Telemed will charge any fees associated with your User Account to your credit card or debit card on file with Telemed, as identified in your User Account, in accordance with these Terms of Use. Invoices or receipts for payments will be emailed to you or available through your User Account. You represent and warrant that (i) the credit card or debit card information you provide to Telemed is true, correct and complete, and (ii) that you are the person in whose name the credit card or debit card was issued and/or you are authorized to make a purchase with the relevant credit card or debit card. You will promptly notify Telemed if your payment information has changed, if your payment method has been canceled, or if you become aware of a breach of security. You acknowledge that we may process an authorization hold using your payment information in order to verify the information provided. If your payment card details change or are due to expire, we may request updated payment details from you, including your card number, expiration date, and CVV (or equivalent). By providing us updated payment information, you authorize us to continue to charge your card using the updated information so that you can continue to receive your Subscription or other Services. 

If any invoice is not paid in a timely manner, or your payment is not successfully settled, due to expiration, insufficient funds, charge backs, or otherwise, we reserve the right to suspend, disable, cancel or terminate your Subscription (as defined below) or User Account until we have successfully charged a valid payment method.

Other Fees. 

You agree to pay all other fees and charges associated with your User Account that are not included in the Subscription Fee, including, for example, appointment no-show fees, cancellation fees or By using the Services and incurring such Other Fees, you authorize us to bill and charge your payment method on file for such Other Fees in full. 

Telemed has the right to amend these Terms and Conditions at any time without notice to you by posing the revised Terms and Conditions on this Site. You agree that you are bound by those changes by continuing to use the Site.
